You must make an appointment for

rent or mortgage assistance.

Leave your full name, phone number, zip code, and email address. We will call you back as soon as possible. 

For RENT you must provide:

  1. Copy of current signed lease
  2. Past Due statement or Accounting statement showing
  3. the amount of rent owed
  4. Landlord’s W-9 form


IF YOU ARE IN THE COURT SYSTEM FOR EVICTION, WE CANNOT HELP YOU.

For MORTGAGE you must provide:

  1. Copy of your most recent mortgage statement
  2. If mortgage is an individual, we will need their W-9 


TO SCHEDULE THE APPOINTMENT

You must have the documents available in hard copy, NOT ON YOUR PHONE.

Bring someone with you to translate as we don’t always have a bi-lingual interviewer available.

In the email, include your full name, phone number, date of birth and last 4 numbers of your Social Security.

Once you have sent the email, call the hot line number again and leave a message that all the paperwork is submitted OR you have all the documents in your hands and are ready to schedule an appointment.
