You must make an appointment for

rent or mortgage assistance.

For RENT you must provide:

Copy of current signed lease

Past Due statement or Accounting statement showing

the amount of rent owed

Landlord’s W-9 form

IF YOU ARE IN THE COURT SYSTEM FOR EVICTION

WE CANNOT HELP YOU

 

For MORTGAGE you must provide:

Copy of your most recent mortgage statement

If mortgage is an individual, we will need their W-9

 

TO SCHEDULE THE APPOINTMENT

You must have the documents available in hard copy,

NOT ON YOUR PHONE.

In the email, include your full name, phone number, date of birth and last 4 numbers of your Social Security.

 

Once you have sent the email, call the hot line number again and leave a message that all the paperwork is submitted OR you have all the documents in your hands and are ready to schedule an appointment.
