1. Copy of signed current lease including monthly rent amount
2. Statement of the amount of rent past due signed and dated by the landlord (if rent is past due)
3. W9 form completed and signed by the landlord within the last 6 months.

1. Copy of most recent Mortgage Statement showing amount of monthly payment, lender name and address, mortgage holder name and address.
